Home Care Support After Hospital Discharge – Essential Information

For elderly patients being discharged from hospital or a nursing home, receiving appropriate home care support is vital for recovery and maintaining independence. In Brighton & Hove and East Sussex, initial home care following discharge can be provided free of charge for up to six weeks, funded either by the NHS or the local council, depending on the circumstances and specific eligibility criteria.

However, delays in arranging these care packages are common due to high demand, leading to extended hospital stays – a phenomenon known as bed blocking. This can negatively impact a patient’s health, increasing the risk of infections and causing physical and mental deterioration.

Limitations of Initial Free Care Packages

The initial care package typically consists of short 15-minute visits, which are often insufficient to provide comprehensive support, particularly for those with complex needs. Furthermore, these visits may only be available for a limited period of up to six weeks, after which a reassessment is conducted to determine ongoing care needs and funding eligibility.

To qualify for further funded care, individuals must undergo a needs assessment conducted by adult social services, which considers factors such as health conditions, mobility, and financial circumstances. This assessment will determine the level of care required and whether it will be funded by the council or if a contribution from the patient is necessary.
