What is End of Life Care? 

End of life care provides comfort, dignity, and support to individuals who are approaching the end of their lives. The care meets the unique needs of the individual and their family during this very sensitive time. End of life care emphasises the importance of respecting patients' wishes while staying at home. The aim is to support the individual and their family in a  nurturing and peaceful way that allows individuals to live as comfortably as possible in their final days, surrounded by their loved ones. A detailed care plan will be developed, based on the individuals specific requirements. A small team of 2-4 carers will attend to the individuals needs, ensuring a trusting and honest relationship is maintained throughout the period of care.
