What Does It Mean to Evolve Care?
As people grow older, recover from illness, develop new conditions, or experience life changes, their care needs can shift — sometimes gradually, sometimes suddenly.
Evolving care means:
Reassessing care plans regularly to reflect changing needs, routines, and goals.
Listening to the individual and those close to them to understand how they feel and what matters most.
Respecting changing beliefs, wishes, and priorities, whether related to faith, culture, routines, or end-of-life choices.
Responding quickly and flexibly when something new arises — a fall, a hospital discharge, or simply a new preference for how care is delivered.
Regular Reviews: More Than a Tick-Box
Every Amaiva client has a personalised care plan — and we treat it as a living, breathing document.
We:
Review care plans regularly with the individual and their family
Involve carers in highlighting any changes they observe
Act quickly if a client’s condition improves or declines
Work with health professionals when needed to make joined-up decisions
But most importantly, we listen. Because no one knows what someone needs better than they do.
Person-Centred Means Present-Centred
People are not the same from month to month — or even day to day.
Being truly person-centred means recognising that:
A client may want more independence as they regain strength
Someone's cultural or spiritual needs might become more important as they age
A loved one’s voice may need to be heard more during times of uncertainty
End-of-life preferences may change over time, and that’s okay
We build flexibility into everything we do—because care should be led by the person receiving it, not the system around them.
Real-Life Example of supporting people as their care needs change
One of our clients, initially recovering from a fall, needed intensive support for mobility and medication. Over time, as her health improved, she expressed a desire to cook for herself again and reduce her care hours.
We didn’t just celebrate her progress—we adapted the care plan, introduced independence-focused visits, and adjusted rotas accordingly.
